Supporting the transition to Adulthood at MDC:
With new interventions and therapies, the need for transition from pediatric to adult care is growing and so is the need for adults affected by neuromuscular disorders (NMDs) to have access to equipment, assessments and assistive technologies to continue to enhance their quality of life.
MDC is committed to supporting people affected by NMDs at all stages. As the needs of this group continue to evolve, MDC has made it a priority to:
- bring the adult community together to share and learn from each other
- support multi-disciplinary partnerships in health care and transitional care delivery
- offset expenses for assistive devices, home and vehicle modifications and assessments, advocacy, education and help navigating complex systems for adults affected by SMA
- influence policy change so that sustainable holistic health care and community supports are in place to assist with unique complex needs
- connect adults affected by neuromuscular disorders with a dedicated Service Specialist to help navigate complex systems and find support

If I Knew Then What I know Now: Transition to Adulthood Conference
In November 2021, MDC hosted an online one-day conference – “If I Knew Then What I Know Now: A Focus on Transition to Adulthood” focused on making the transition from pediatric to adult care health care and community settings.
The engaging virtual conference was co-chaired by Jared and Tori – two individuals living with Spinal Muscular Atrophy – and had panel members including researchers and healthcare professionals from across the neuromuscular community.
